The Family Nurse Practitioner (FNP) will serve as a primary care provider delivering in-home medical services to patients across Northern Virginia. The ideal candidate will be independent, detail-oriented, and compassionate, capable of managing a diverse patient population with acute and chronic medical needs.

This role is ideal for an experienced, new graduate and highly motivated FNP who enjoys flexibility, and meaningful patient relationships in a home-based primary care model.

Essential Responsibilities

Conduct comprehensive in-home health assessments, physical exams, and follow-up visits.

Diagnose and manage acute and chronic conditions.

Order, interpret, and act upon laboratory and diagnostic tests.

Prescribe and manage medications and therapies per state and federal regulations.

Develop and maintain individualized care plans and coordinate with interdisciplinary care teams.

Educate patients and caregivers about disease management, prevention, and lifestyle changes.

Document patient encounters accurately and timely in the Electronic Health Record (EHR).

Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure continuity of care.

Participate in quality improvement initiatives to promote safety, efficiency, and patient satisfaction.
