Thriving clinic environment, and providers see 12–18 patients per day. Conveniently located off Hwy 41 in the Fox Valley. Radiology, Lab, Physical therapy and Urgent Care onsite.

Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screenings, immunizations and patient care.Provides direct treatment and management of health conditions via referral to other healtcare providers or community resources.Counsels and educates patients and their families / caregivers concerning preventative health, treatment options and community resouces.Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.About Ascension Medical Group

Requirements
Licensure / Certification / Registration :
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Verified by local credentialing office.Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Minnesota Board of Nursing is also accepted. Licensure is dependent on state associate works in. Verified by local credentialing office.Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Minnesota Board of Nursing OR current home state license, if considered multi–state / Compact State, also accepted. Licensure is dependent on state associate works in. Verified by local credentialing office.Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred. Verified by local Credentialing Office.Education :
