REGISTERED NURSE (RN)- Outpatient Clinic

Western North Carolina Community Health Services (WNCCHS) provides patient centered, team-based primary care to residents of Western North Carolina. We are looking for a full-time, 40 hour per week, RN, to join our growing team!
The Staff Nurse- Float is a position that will provide coverage within various treatment teams at the same clinic. These services are needed at our primary location Minnie Jones Health Center, 257 Biltmore Ave, Asheville, NC 28801 . The position would be scheduled to work (4) ten-hour days weekly.
RESPONSIBILITIES : Patient Care
- Obtain a history of presenting health issue(s).
- Performs a nursing duty within the RN (Registered Nurse) scope of practice. (i.e., Performing blood pressure checks, INR management, medication reconciliations) considers the patient's psychosocial, emotional, ethnic, cultural, and spiritual dimensions of health, including his or her understanding of their health / illness experience and how their daily life is affected.
- Uses and adapts assessment tools and techniques based on the patient's unique needs.
- Documents health assessment and clinical data in the visit note.
- Initiates and / or contributes to developing a health plan with the patient and other members of the interdisciplinary team.
- Self-management support - providing information and counseling to help patients set goals and undertake specific activities to self-manage their health and illnesses.
- Medication management - evaluating and monitoring medication adherence (e.g., fill history)
- Emotional support - monitoring patients' psychosocial state and linking patients and / or their caregivers to mental health and community-based supportive services.
- Handles incoming phone calls, phone triage and returning calls to patients related to test results. Documents this care in patient record according to organization policy.
- Assesses health education requirements and patient readiness as part of an overall health assessment.
- Assesses current knowledge, education and literacy levels, social supports, learning preferences and other factors that may affect the education approach and plan.
- Acquires, develops, and evaluates teaching materials and tools, with consideration for cultural, language, physical, intellectual, and environmental factors.
- Employs education strategies and motivational interviewing skills to support behavior changes to enhance health (e.g., smoking cessation, physical activity, and healthy eating).
- Provides education related to healthy living, prevention of injury, illness and communicable diseases, care and treatment, individual and family adjustments, and support systems as appropriate to the patient situation.
- Engages patients in education regarding chronic illnesses such as diabetes, chronic obstructive pulmonary disease, hypertension, cardiac disease, cancer, and chronic pain.
- Utilizes all health-care encounters as an opportunity to identify educational needs and provide health education (e.g., immunizations, screening, health lifestyle, smoking)
- Develops and provides a variety of educational approaches to address health topics, including individual and group sessions, and offers these in a variety of settings to enhance accessibility.
- Utilizes a variety of communication strategies, e.g., written, verbal, non-verbal, visual, internet, and group education.
- Completes tasks as assigned by Providers.
Miscellaneous
Participate in quality improvement activities, including development of clinical forms and protocols and attendance at quality assurance / quality improvement meetings.Occasionally, the supervisor may assign other appropriate duties and responsibilities.QUALIFICATIONS :
Minimum of two (2) years' experience in an outpatient setting.Excellent communication and interpersonal skillsAbility to work independently and as part of a teamAbility to prioritize and manage multiple tasksProficient in Microsoft Office programsSatisfactory completion of educational program clinical rotationsPreferred Qualifications :
Expert knowledge of ambulatory, medical office operations.Familiarity with role of FQHC in the community.Bilingual in Spanish preferredREQUIREMENTS :
Current and unrestricted RN or LPN License in North CarolinaGraduation from an accredited education program for Registered NursesCurrent BLS / CPR certificationFlexibility to work with different pods at same clinic as neededWORK / BENEFITS :

Hourly $27.00 to $38.00 depending on experience and certification.
