Position Summary :

We are seeking a licensed Physical Therapist (PT) to join our in-house rehabilitation team. This role will focus on providing high-quality physical therapy services to residents in our subacute rehabilitation unit, helping them achieve their highest level of function and independence. This position offers flexible scheduling with full-time and per diem opportunities available.

Key Responsibilities :

Evaluate, plan, and implement individualized physical therapy programs for residents.

Provide therapeutic interventions, including mobility training, balance exercises, strengthening, and gait training.

Monitor and document resident progress, updating care plans as needed.

Collaborate with physicians, nursing staff, occupational therapy, and speech therapy to ensure coordinated care.

Educate residents and families on exercises, adaptive techniques, and safe mobility.

Participate in team meetings and quality improvement initiatives.

Maintain compliance with federal, state, and facility regulations.

Qualifications :

Licensed Physical Therapist in the state of Iowa.

Minimum of 1 year of experience in a skilled nursing or subacute rehabilitation setting preferred.

Strong clinical reasoning, communication, and documentation skills.

Ability to work independently and as part of a multidisciplinary team.

CPR certification required or willingness to obtain.

Work Schedule :

Full-Time : Monday–Friday, standard business hours (flexibility may be required).

Per Diem : Flexible scheduling based on facility needs.
