They will develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability. The therapist will work with the individual to prevent the loss of mobility. Essential functions and responsibilities include performing patient evaluations through the process of obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data. They will establish an individualized plan of care and select treatment techniques that address the specific impairments identified in the evaluation and in accordance with physician orders. Ongoing reassessment of the patient and their response to treatment will be conducted. The therapist will initiate and manage the discharge plan beginning at the first visit and continuing throughout treatment. Supervision and delegation to ancillary staff and students will be consistent with state laws and department policy. They will complete legible, accurate, and clear documentation within the established timeframe identified in department policy and procedure and in accordance with accreditation requirements and insurance regulations. Communication with the patient, family, caregivers, significant others, and members of the healthcare team will be necessary to promote maximum benefit of care. The therapist will assess the educational needs of the patient, family, caregiver, or significant other and provide education to meet those needs. Participation in professional growth and development through educational programs, including in-service meetings, workshops, and reading current literature concerning the scope of practice is expected. Active participation in quality improvement activities is required. The therapist will provide an environment conducive to safety for patients, visitors, and staff. They must comply with appropriate and approved safety standards. The physical therapist will function within the scope of practice defined by the state’s practice act. Other related duties may be performed as required and directed.
