Job Description & Requirements Physical Therapist - Home Health - (PT - HH) StartDate : ASAP Available Shifts : 8 D Pay Rate : $1867.81 - $2046.64 Pt- home health needed asap Performs a comprehensive Physical Therapy assessment of patients referred for service including but not limited to physiological, psychosocial, environmental factors and health history. Integrate pertinent data to develop a plan of care to promote function and safety. Participates with other staff in the development of agency policies and programs necessary to achieve agency objectives as they relate to patient care. Bachelor of Science Degree or Master's Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy Association.

• Current PT license in Mass. with one (1) year clinical experience and CPR certification required.
• Computer skills preferred.
