Role Description

This is a full-time, on-site Nurse Practitioner role located in Lincoln, NE. The Nurse Practitioner will provide high-quality medical care to patients primarily in their homes, assisted living, memory care, or independent living communities. Daily responsibilities include performing patient assessments, diagnosing and treating acute and chronic conditions, developing and managing individualized care plans, prescribing medications when necessary, and collaborating with other healthcare professionals. The role also involves conducting post-acute follow-up visits and educating patients and their families on health management and preventive care strategies.

Qualifications

• Advanced clinical skills, including patient assessment, diagnosis, and treatment, with the ability to manage acute and chronic conditions.
• Proficiency in creating and executing individualized care plans and prescribing medications as necessary.
• Strong communication, interpersonal, and patient education abilities to effectively interact with patients, families, and other healthcare professionals.
• Organizational and time management skills capable of coordinating care across multiple patients and settings.
• Current Nurse Practitioner licensure in the state of Nebraska and national board certification.
• Experience in geriatric care, home health, or primary care is highly desirable.
• Knowledge of electronic medical records and documentation systems is an advantage.
