Licensed Practical Nurse (LPN) – Job Description
Position Summary : The Licensed Practical Nurse (LPN) provides primary nursing care to residents under the direction and supervision of the attending physician or Medical Director. This role focuses on assessment, illness prevention, and health care management. The LPN supports the development and modification of treatment plans to meet residents' physical and psychosocial needs in accordance with established medical practices, state regulations, and facility policies.
Essential Duties and Responsibilities :
- Collaborate with the attending physician and Medical Director regarding resident care.
- Assist in planning and developing nursing services based on resident evaluations.
- Initiate and respond to consultation or referral requests from staff, residents, or physicians.
- Review resident charts and assessments to identify health changes and respond appropriately.
- Refer residents to physicians for further evaluation or care as needed.
- Complete and maintain accurate medical forms, records, evaluations, and reports.
- Implement and uphold facility policies, procedures, and safety standards.
- Communicate care updates and concerns with physicians, administrators, and families.
- Participate in planning and evaluating nursing service procedures and policies.
- Support facility compliance during state and federal surveys and inspections.
- Address staff complaints and ensure fair employment practices within the nursing department.
- Assist in managing infection control protocols and ensuring proper use of PPE.
- Participate in resident assessments and care planning processes.
- Observe medication administration and treatments for quality assurance.
- Perform nursing procedures within scope of practice (e.g., catheter care, dressing changes, tube feeding, etc.).
- Administer prescribed medications and monitor for effectiveness or adverse reactions.
- Review medication records for accuracy and compliance with policies.
- Document all care provided, changes in condition, and communications with providers in a timely and professional manner.
- Maintain adequate stock of medications and medical supplies.
- Assist with Quality Assurance initiatives and corrective action planning.
- Perform other related duties as assigned.
Qualifications :
Education and Experience :
High school diploma or equivalent.Graduate of an accredited Practical or Vocational Nursing program.Licensure and Certifications :
Current and active LPN or LVN license in good standing for this state.TB clearance as required by facility policy.CPR certification preferred.Knowledge, Skills, and Abilities :
Ability to read, write, speak, and understand English clearly.Ability to make independent decisions and work under pressure.Strong interpersonal and communication skills with residents, staff, and families.Knowledge of nursing practices, medical procedures, and regulatory requirements for long-term care.Leadership ability with willingness to guide and support team members.Patience, empathy, and the ability to work with diverse resident populations, including those with challenging behaviors.Work Environment :
Works in various nursing service areas including resident rooms and nurses' stations.Regular interaction with residents, families, staff, and outside personnel.Subject to occasional interruptions, emotionally demanding situations, and exposure to infectious materials and hazardous chemicals.May work weekends, holidays, or be on-call during emergencies.Physical and Sensory Requirements :
Must be able to move intermittently throughout the day.
