Job description

Summary

Under the limited supervision of the Clinic Supervisor, the Nurse Practitioner performs a variety of clinical and public health nursing responsibilities. The primary role is to provide services to clients. This includes oversight of the Reproductive Health Program, in-house pharmaceuticals, laboratory functions, and onsite pharmacy and dispensing practices. A person in this position is expected to work in a highly independent manner with utmost integrity and reliability to ensure quality of health care delivered is in accordance with federal, state and county policies and procedures. This position requires the commitment to continually improve quality of care and services provided by the health department by attending as needed trainings and participating on the Quality Team.

Clinic Services

  • Perform appropriate complaint related and annual wellness exams for all patients regardless of ethnicity, gender, religious beliefs, or language spoken
  • Offer appropriate medical plan of care and collaborate with client to determine final plan of care, labs, and prescriptions
  • Knowledgeable of all federal, state and local clinic program policies and procedures
  • Assures the effective and efficient delivery of clinical service in accordance with high quality standards of care, and applicable federal, state and local requirements.
  • Monitors client satisfaction and follows up with dissatisfied clients.
  • Establishes and maintains community partner relationships.

Training and documentation

  • Perform chart audits and monitor chart audit results collaborating with Clinical Supervisor for needed action plans
  • Train staff on lab and pharmacy competencies as required.
  • Educates community on health via multi-media outlets and events.
  • Documents timely and thoroughly utilizing appropriate LOS codes.
  • Laboratory and Medicine

  • Assure quality of daily lab functions and cleanliness as needed.
  • Monitor lab QI metrics and report on regular basis to CCHHS Leadership and QI
  • Coordinate, manage, and implement federal, state and local guidelines for laboratory and pharmaceutical licensure
  • Perform inventory and ordering of medications and supplies as needed.
  • Demonstrate proper specimen handling, processing, testing, charting and disposal.
  • Demonstrate proper use of lab equipment, testing mediums and processes.
  • Demonstrate proper techniques and documentation for all procedures and dispensing medications.
  • Create, implement, and maintain Clinic, Lab, and Pharmacy policies and procedures.
  • Train new staff on lab competencies as needed.
  • Immunization

  • Understand terms and concepts associated with childhood and adult immunization and the diseases they prevent
  • Describe and apply current Oregon Health Authority Immunization Schedule for specific ages
  • Understand department policies and procedures for administering vaccinations
  • Accurately interpret Oregon School Law and implement standards for sites required to report
  • Interview each parent / client for possible precautions and contraindications specific for each vaccine and possible reactions as needed.
  • Vaccine Administration

  • Describe and demonstrate proper vaccine storage and handling
  • Administer vaccine in a safe manner, using universal precautions and sharps safety according to standing orders and procedures
  • Demonstrate safe child comfort restraint and immunization technique per department policy
  • Accurately describe procedures for immunization-related emergencies
  • Documentation

  • Complete all forms, consents, records, and other documentation required by the County and OHA
  • Competently document in electronic medical record software
  • Understand terms and concepts involved in determining payer source for vaccine with local, state and federal programs or private insurance
  • Family Planning

    Coordination of Family Planning, STD, HIV Prevention, and Lab programs.

  • Assure the effective and efficient delivery of clinical service in accordance with high quality standards of care, and applicable state and federal requirements.
  • Ability to provide IUD insertion and removal.
  • Ability to provide Implanon insertion and removal.
  • Diagnosis and treat reproductive health problems. Evaluate and refer appropriately.
  • Diagnosis and treat STD's. Evaluate and refer appropriately.
  • Coordinate day-to-day clinic operations for efficiency in flow, client management issues, chart documentations and adhere to State, Federal and County regulations, policies and procedures
  • Work with clinical staff to conduct education / training classes and staff meetings for Programs.
  • Discuss, review and resolve complaints from clients, other agencies and the public.
  • Assist in the preparation of State, Federal and County reports with Leadership as needed.
  • Coordination and updating of Nursing standards of Practice.
  • Develop and maintain effective, harmonious and reasonable work relationships with others.
  • Maintain regular and predictable work attendance. May be asked to work late hours and weekend hours.
  • Understands terms and concepts associated with family planning program policies and procedures.
  • o Understands terms and concepts associated with laboratory policies and procedures.

    o Describes and applies current Oregon Health Authority policies and procedures regarding family planning services.

    o Interviews each client for possible precautions and contraindications specific for each birth control method and potential side effects.

  • Review and update clinic policies and procedures as needed.
  • Update forms as needed.
  • Orders contraceptive supplies as needed.
  • Complete annual family planning plan.
  • Assess no-show rates and improve processes as needed.
  • Conducts clinic services including options counseling, contraceptive counseling, STD exams, and medical assessments.
  • As a member of local coalitions, plan local events for the public, as applicable
  • Educate staff on Reproductive Health and medical consent laws and new services
  • Medication

  • Describes and demonstrates proper dispensing of pharmaceuticals.
  • Administers medications in a safe manner using universal precautions and sharps safety according to standing orders and procedures.
  • Describes procedures accurately for clinic related emergencies.
  • Documentation

  • Completes all forms, consents, records, and other documentation as required by Crook County and Oregon Health Authority.
  • Competently uses electronic medical record documentation process.
  • Understands terms and concepts involved in determining payer source for family planning services with local, state and federal programs or private insurance.
  • Public Health Preparedness :

    1. Public Health Preparedness Training : IS 100, 200, 700, 800.

    2. Participate in Public Health Preparedness exercises and events.

    Supervisory Responsibility

    This position has no administrative supervisory responsibilities. Provides consultation for RNs

    Work Environment

    Ability to work in a fast-paced environment, work evenings and weekends for events occasionally, and travel when required; this role routinely uses standard office equipment such as computers, phones, and photocopiers. Regular meetings with community partners.

    While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit; stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms. Ability to lift up to 20 pounds, twist, and bend.

    Position Type and Expected Hours of Work

    This can be a full or part-time position. Typical days and hours of work are Monday through Friday, 8 : 00a.m. to 5 : 00p.m. Some weekends and evenings will be required for travel, events, and / or training. Days and hours may vary in a public health emergency. This position qualifies for limited flexible scheduling options as approved by the department Director.

    Travel

    Travel throughout Central Oregon to meetings, and occasional State or National meetings. Must hold a valid Oregon's driver's license.

    Required Education and Experience

  • BSN from an approved school of nursing and graduate level preparation as a nurse practitioner licensed in the State of Oregon (Master's Required)
  • National Certification from American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
  • Eligible for Drug Enforcement Administration (DEA) and National Provider Identifier (NPI) certification
  • Preferred Education and Experience

  • Doctor of Nursing Practice
  • Current DEA and NPI certification
  • Current Basic Life Support (BLS) certification
  • 1+ year clinical experience in addition to requirement for licensure
  • Previous experience in Women's Health procedures
  • Additional Eligibility Requirements

  • Must hold a valid Oregon driver's license
  • Able to complete a thorough employment and / or criminal history check.
  • Final candidates will be required to provide official proof of college degree and certified transcripts (not required for all positions).

    This position is considered to be a safety sensitive position, subject to drug screen(s) and adherence to all vaccine / immunity requirements for the healthcare setting.
