Position Overview The Registered Nurse - Home Health provides coordinated, high-quality nursing care to patients in their place of residence. This position is responsible for assessing patient needs, developing individualized care plans, coordinating with multidisciplinary teams, and ensuring optimal outcomes through compassionate, evidence-based care. Key Responsibilities Conduct comprehensive assessments and develop individualized care plans using the nursing process. Deliver direct patient care and coordinate multidisciplinary services to ensure continuity of care. Evaluate patient progress and adjust interventions as needed. Provide health education to patients and families, ensuring understanding and engagement in care plans. Complete all OASIS and clinical documentation accurately and in a timely manner. Supervise and delegate duties to other nursing staff, maintaining a collaborative and efficient team environment. Promote safety, wellness, and independence in the home setting.
