• NOTE : This position is a hybrid remote position with up to 10% travel. Preference for individuals that have (1) an active Illinois RN license, (2) past experience working with child welfare or youth foster care or post adoption families and (3) reside in one of the following counties : Adams, Hancock, Pike, Calhoun, Jersey, Greene, Scott, Brown, Schuyler, Morgan, Cass, Menard, Mason, Sangamon, Macoupin, Montgomery, Christian, Logan, De Witt, Macon, Piatt, Moultrie, Shelby, Douglas, Coles, Cumberland, Clark, Champaign, McLean, Livingston, Ford, Edgar, Vermilion, Iroquois.

Additional Details :

Position Purpose : Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families.

Education / Experience : Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 – 4 years of related experience.

License / Certification :

Pay Range : $55,100.00 - $99,000.00 per year
