Adding Exceptional Family Nurse Practitioner to the team

This is not entry level - minimum of 3 years primary care practice experience required.

Our team of 12 providers -MDs and NPs - are looking for an exceptional Nurse Practitioner who can handle a high volume practice with minimal oversight.  The need is for a seasoned and dedicated primary care provider who can start, ramp up quickly, take over a panel and increase our patient access in this premier practice.

Patterson Avenue is a "Primary Care First" practice - one of only 960 in the U.S. -  a value-based care model which prioritizes outcomes and access to care.

• 3 years mininum Outpatient Ambulatory Primary care NP practice is required
• Virginia FNP License Required.
• FNP only - Must be able to treat all ages - peds age 5+ through geriatric
• 15% peds, 50% adult, 35% geriatric
• Schedule is full time - M-F 8a-5p
• Call rotation is shared by practice providers - 1 day out of every 12 days - Rx refills on call are provider discretion, no scheduled drugs or pain meds
• 18-20 patients per day average
• 1 : 1 CMA or LPN per provider - 1 RN Clinical Supervisor
• Telemedicine and E-visits through MyChart will be a part of your practice
• Basic in-office procedures incl. women's health, joint injections, I&D, occasional sutures, basic derm
• EPIC EMR
• X-ray and Lab in practice
• LCSW, RN Care Manager, PharmD, Community Health Worker, Radiology and Phlebotomist and P.T. are in the same building with the practice.
