POSITION PURPOSE :
The Community Support Service Coordinator is responsible for coordination of person-centered services for individuals and assisting them to successfully living independently in the community by providing support services, coordinating outside supports, and working in collaboration with landlords, case managers and / or other team members. The Community Support Service Coordinator manages various social service tasks to facilitate individuals to meet their desired objectives with regard to housing, personal assistance, equipment and / or medical services. This includes aggressively seeking new housing partnerships, coordination and development of Housing Support programs and the development and implementation of new housing programs / services.
NATURE AND SCOPE :
The Community Support Service Coordinator reports to the Director of Community Supports who reports to the Executive Vice President, who reports to the President. The Community Support Service Coordinator is responsible for a variety of individual tasks, which require knowledge of housing programs, funding sources and regulations, knowledge of representative payee responsibilities and requirements and systems to manage this, and knowledge of various disabilities, chemical dependencies and mental health diagnosis. The Community Support Service Coordinator will supervise support workers. The Community Support Service Coordinator must have the skills and knowledge to navigate complex service delivery systems and partner with other agencies and team members. Excellent interpersonal relationship skills are required. The job requires frequent contacts with clients, property owners and managers, employees, families, guardians, legal representatives, financial workers and other members of the interdisciplinary team to coordinate services for individuals. The work environment requires frequent driving, walking, standing, and sitting. Travel is required. The Community Support Service Coordinator participates in frequent phone, deskwork and computer work.
The Community Support Service Coordinator is responsible for the facilitation and oversight of the admission process into housing programs. Maintaining and adhering to county contracts. The Community Support Service Coordinator will work with individuals to identify appropriate service options and will aggressively work to develop
partnerships in the community to creatively serve individuals as they desire. The Community Support Service Coordinator will need to maintain a clear understanding of housing programs and service options and be able to discuss them with potential service recipients.
JOB RESPONSIBILITIES :
1. CLIENT SERVICES and Management of Client Services- Assists clients in locating and gaining access to housing and necessary services, as well as medical, social, educational, or other needed services regardless of the funding source; coordinates plan of care; acts as a liaison between client and partnering agencies, including landlords; monitors services so the client's health and safety are assured; provide on-call support and responds to and problem solves potential crisis situations. Completes representative payee duties, including payment of rent and utilities, coordination of monthly statements and coordination with financial department to assure accurate accounting of all housing and / or social security funding. Participates as an interdisciplinary team member, functions as an advocate in relating client grievance issues. Sits and writes and / or types frequently in order to prepare reports, forms, or information related to overall plan of care.
2. CUSTOMER / HUMAN RELATIONS- Functions as a role model, communicates effectively with clients, families, co-workers, landlords, case managers and outside agencies. Ability to work with clients with a variety of cultural, medical, psychiatric, intellectual and physical needs. Displays positive, supportive, and cooperative conduct. Maintains and fosters cordial relationships and optimal communication. Acts as a role model in the appropriate conduct for meeting client needs. Communicates effectively verbally and in writing; listens accurately and responds appropriately.
3. REGULATION COMPLIANCE- Adheres to and maintains knowledge of various rule, regulations, and state and county policies regarding the Home and Community Based Waiver, including DD, CADI and TBI waiver programs; county Housing Support regulations, requirements, and financial resources; Medical Assistance; Homeless Management Information Systems data entry; RPM software and other systems to manage client services. Completes all necessary documentation. Develops systems that provide continual improvement in quality services.
4. SAFETY- Monitors and responds to emergencies according to established policies and guidelines. Assesses client risk and responds appropriately to maintain client safety. Maintains an injury free workplace by identifying and reporting unsafe working conditions.
5. DEVELOPMENT OF POSITION : Participates in on-going education in topics related to diagnoses of the individuals served; legislative and statutory changes and updates pertaining to services and funding streams; and regulation changes impacting client services. Attends and completes all annual in-service requirements. Provides training to employees as necessary.
6. ASSUMES ADDITIONAL RESPONSIBILITIES- Identifies and responds to client needs in emotional and social areas, however, intervention in these areas does not fall within the meaning of psychotherapy. Performs tasks which may fall within Category II of infection control; there is no regular exposure to blood or high-risk body fluids.
