About the job Home Health Physical Therapist

Overview

We're hiring a compassionate, self-motivated Physical Therapist to join a top-tier home health team serving patients across Lansing, Michigan. In this role, you'll deliver patient-centered care in the comfort of patients' homes, supported by dedicated technology, flexible scheduling, and career development resources.

Key Responsibilities

  • Provide skilled physical therapy treatment to home health patients
  • Collaborate with interdisciplinary care teams to implement patient care plans
  • Maintain accurate, timely clinical documentation in the EMR system
  • Communicate with patients, families, and other healthcare providers
  • Utilize company-provided tablet and smartphone to complete documentation efficiently

Required Qualifications

  • Education : Bachelor's degree or higher in Physical Therapy from an accredited institution
  • License : Valid Michigan Physical Therapist license
  • Experience :

    • 1+ years of healthcare experience

  • 1+ years of experience with any of the following :

    • Electronic Medical Records (EMR)

  • TruChart EHR or other EMR systems
  • Certifications : Valid Driver's License (DL)
  • Must not have applied to or worked for Residential Home Health & Hospice in the past 6 months
  • Must have spoken with a Relode recruiter about this position
  • Preferred
  • Please provide your MI PT License Number when applying
