What We're Looking For

Trillium Health Resources has a career opening for a Population Health Nurse Supervisor to join our team! The Population Health Nurse Supervisor is responsible for the administrative oversight and clinical supervision of the Population Health Nurses who provide care management and care coordination services, linkage / referrals, and discharge planning activities. Reporting to the Population Health Nurse Manager, the Population Health Nurse Supervisor is available to licensed and non-licensed staff to perform clinical triage, lead on emergency or crisis calls / contacts, or provide guidance and support, as needed.
