GENERAL SUMMARY
Under direction, completes Omnibus Budget Reconciliation Act (OBRA) pre-admission (PAS) and annual resident review (ARR) screenings. Delivers and reviews completed screenings with consumers and guardians.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Reviews and prescreens Level I referral forms submitted by hospitals and nursing homes to determine need for full assessment.
Schedules Level II Assessment with nursing facilities, hospitals, consumers, guardians and family homes.
Completes face-to-face assessments at nursing facilities, hospitals and family homes. Consults with nursing facility and hospital staff. Reviews available clinical records to incorporate into Level II assessment.
Completes Level II Assessment in State EMR system.
Reviews list of Level II assessments with upcoming due dates. Notifies nursing facilities of assessments coming due.
Delivers and reviews completed Level II Assessments with consumers and guardians.
Screens Level II assessments for consumers needing specialized treatment and transition services. Makes referrals for specialized services or other mental health services.
Performs indirect clinical services, including, timely and complete case documentation following Macomb County Community Mental Health (MCCMH) policies and procedures; confirms diagnosis to make the most suitable, least restrictive, and medically necessary clinical recommendations; and assists client and / or family in obtaining appropriate programs and resources.
Maintains accurate clinical documentation, including, assessments, progress notes, treatment plans and other required documentation.
Ensures administrative paperwork is accurate and up to date, including, staff activity logs, statistical reports, attendance, mileage reports, and case lists.
Participates in agency and system staff meetings.
Conducts thorough, accurate, clinically sound triage / screening with consumer / guardian / family members in a timely manner.
Requests appropriate authorizations for services provided; monitors utilization of services; requests re-authorizations or continuations as appropriate.
Serves on community mental health or community-based committees as directed.
May assume the administrative responsibilities of a Clinical Supervisor in their absence.
May supervise the work of department interns.
Provides support and assistance to OBRA Coordinator as needed.
Performs related duties as assigned.
