GEM Transitional Care Center is a 75 bed skilled nursing facility specializing in short-term therapy and dedicated to providing all of our residents with the type of care they deserve. We are looking for a full time Speech Language Pathologist to join our growing in-house team.
We offer a variety of therapies and care services to assist patients in regaining their independence, whether they are recovering from a hospital stay or require ongoing medical support. As the Speech Language Pathologist and a valued team member, you will collaborate with therapists, patients, their families and the IDT to guide each patient through a personalized therapy program, helping them reach their recovery goals.

REQUIREMENTS :
CPR Certification (Required)Licensed Speech Language Pathologist (Required)1-2 years' experience in long-term care preferred.Good physical and mental health.The candidate must be able to perform any physical requirement of the position, including lifting dead weight or an unassisted weight of 40 pounds.Is capable of following oral and written instructions.Have training, knowledge, and experience, which allows for general supervision of responsibilities.REPORTING : TheSpeech Language Pathologist Reports to the Rehabilitation Director
JOB DUTIES :
Basic knowledge of physical medicine, physical therapy modalities, anatomy, and physiology.Treat patient following Speech Language Pathology Standard of Care and Practice.Provides assessment, treatment, and rehabilitative services to patients who have experienced injury or disability disease. Provides residents with exercises related to Speech-language pathologyAdheres to standards of physical medicine practice.Ability to observe and evaluate treatment effect and discuss observations with staff, Physician, and Rehab Director.Responsible for the monitoring and documentation of resident treatment and response.Assists with formulating a teaching plan based upon identified learning needs and assesses learning effectiveness; family is included in teaching as appropriate, and staff and Physician are apprised of conclusions.Meets current documentation standards and policiesAdheres to the facility's Policy and ProcedureIND123
