The Opportunity : As a Field Nurse (RN) with Executive Home Care, you'll be vital to our patients' well-being, providing comprehensive nursing care and support in the comfort of their homes. This role offers professional independence, direct patient impact, and the satisfaction of building meaningful relationships.

Key Responsibilities :

• Comprehensive Patient Assessment : Conduct thorough initial and ongoing assessments of patients' physical, mental, and emotional health, identify needs, and develop individualized care plans.
• Medication Management : Assist with medication administration and ensure proper adherence to prescribed regimens.
• Personal Care Assistance : Oversee and provide guidance on activities of daily living (ADLs), such as bathing, dressing, and feeding, to ensure patient comfort and dignity.
• Vital Signs Monitoring : Regularly monitor and assess vital signs, reporting any significant changes to the appropriate healthcare professionals.
• Health Education & Support : Educate patients and their families on general health and wellness, disease prevention, and self-care techniques to promote independence and optimal health outcomes.
• Documentation & Communication : Maintain accurate, timely, and detailed clinical documentation in electronic health records (EHRs). Communicate effectively with the interdisciplinary team, physicians, and families to ensure coordinated and holistic care.
• Care Coordination : Act as a liaison between patients, physicians, and other healthcare providers to ensure seamless transitions of care and access to necessary resources.
• Emergency Response : Respond to changes in patient condition and medical emergencies, initiating appropriate interventions and seeking medical assistance as needed.
• Adherence to Standards : Adhere to all professional nursing standards, ethical guidelines, and company policies and procedures, including HIPAA regulations.
• Professional Development : Participate in ongoing professional development and continuing education to maintain licensure and enhance clinical skills.
