Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities Administer medications, injections, IV infusions, and perform venipuncture with precision and adherence to protocols Conduct physical examinations, vital sign assessments, and patient evaluations Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing Manage airway care, ventilator support, and respiratory management including oxygen therapy Document all patient interactions accurately using EMR / EHR systems like Cerner or Athenahealth while maintaining HIPAA compliance Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, infectious diseases, or post-acute needs Support discharge planning and coordinate with multidisciplinary teams for case management and follow-up care Educate patients and families on health coaching topics including medication administration, nutrition, pain management, and disease prevention Participate in infection control practices and uphold standards for aseptic technique in all procedures Assist in diagnostic evaluations such as laboratory specimen collection / processing or medical imaging preparations Requirements Valid Licensed Practical Nurse (LPN) license in the state of practice, BLS, Covid Card. Proven experience in diverse clinical settings such as ICU, emergency room, medical-surgical units, hospice care, or long-term care facilities Proficiency with EMR / EHR systems including Cerner or Athenahealth; familiarity with ICD-9 / ICD-10 coding preferred Strong knowledge of physiology, anatomy, pharmacology, and medical terminology Experience administering injections, IV insertion / infusion therapy, catheter care, and tube feeding techniques Ability to perform physical examinations and vital signs assessment accurately Knowledge of infection control standards and aseptic techniques Ability to work independently while collaborating effectively within a healthcare team Excellent communication skills with a compassionate approach to patient care
