We are currently seeking a skilled and motivated Licensed Practical Nurse (LPN) to join our healthcare team. The work will be at an SNF (Skilled Nursing Facility). • Type : Per Diem
• Location : Bellevue, WA Responsibilities :
• Understand and adhere to established policies and procedures.
• Ensure adherence to Universal Precautions for resident and staff safety.
• Prepare residents' medications for short-term absences following state regulations.
• Collaborate with the healthcare team to implement the nursing care plan for each patient.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and symptoms and report to the physician and RN, including reactions to treatments and changes in the patient's physical or emotional condition.
• Teach and counsel patients and their families / significant others on nursing care needs and related issues for home care.
• Participate in medical record audits as assigned.
• Attend staff meetings, patient care conferences, and in-services as scheduled.
• Read Daily Communication Logs for resident information.
• Document any changes in resident status in Progress Notes and review them regularly.
• Complete resident treatments as ordered and scheduled.
• Assist with personal care and physical support as needed.
• Performs other duties as assigned. Other Skills or Qualifications :
• Proficient computer skills in managing the electronic health record (EHR).
• Ability to manage information electronically and transfer records appropriately.
• Ability to work in an electronic environment.
• Refined deductive reasoning skills for problem-solving during shifts.
• Must have a valid driver’s license or dependable access to a vehicle for business use.
• Must be in good health and physically / mentally capable of performing assigned tasks.
• Working knowledge of personal computing and word processing software, particularly Microsoft Windows.
• Completion of appropriate competencies as required by the facility. Requirements :
• Licensure : Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in WA state.
• Active BLS (Basic Life Support) certification.
• Preferred minimum of one (1) year of nursing experience.
• Strong communication and interpersonal skills.
• Ability to work well in a fast-paced SNF environment.
• Two (2) positive work references
