Primary Vascular Care is looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site clinical assessments to determine appropriate patient treatment plans of residents of client facilities. This position reports to the Lead Nurse Practitioner.
The NP / PA will work in a mobile practice, primarily serving a long-term care setting.
This position is 40 hours a week
Functions :
- Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by the completion of a comprehensive assessment form
- Functions independently to perform appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
- Orders and reviews diagnostic and therapeutic tests relative to patient's age-specific needs
- Assists with the provision of care in accordance with facility, state, and federal regulations
- Monitors the effectiveness of interventions
- Maintains relationships with all appropriate contacts in facilities
- Educates family members, clients, and prospective accounts regarding services and programs offered by Primary Vascular Care
- Assists with the development and maintenance of physician relations, as assigned
- Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner
Qualifications :
Bachelor's Degree requiredDegree from an accredited Nurse Practitioner or Physician Assistant Program requiredCurrent NP License requiredBLS - Basic Life Support through American Heart Association requiredACLS - Advanced Cardiac Life Support through American Heart Association preferredCurrent Infection Control Certification requiredBoard Certified / Board Eligible requiredPhysical, Vaccines and Titers : PPD / TB, Hep B, MMR, Varicella requiredCOVID and Flu Vaccine preferred1 year of NP experience preferredPreferred Qualifications :
Experience in Nursing home environment and outpatient vascular clinicsAbility to work independently in a fast-paced environmentCompensation : Competitive and based on experience
