W2 Contract

Typical Work Hours : Days 32+ hours / week

Location(s) : West Columbia, SC

Start Date : ASAP

Practice Setting : SNF (Skilled Nursing Facility)

On-call : No

Minimum Education : PT or DPT

Years of Experience : Any, including New Grad

Float : Never

This job will have the following responsibilities :

• Conduct comprehensive physical therapy assessments and develop individualized treatment plans for patients.
• Implement evidence-based therapeutic interventions to enhance patients' functional abilities.
• Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.
• Maintain accurate and up-to-date patient records and documentation.
• Monitor and report patient progress to healthcare professionals and family members.

Qualifications & Requirements :

• Valid state PT license
• Strong clinical and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
