About The Position

Job Title : Physical Therapist Home Health

Hours per Shift : 8 : 00 AM - 4 : 30 PM

Experience : Minimum 1 year

License : Valid State License required

Certifications : BLS

• Experience with geriatrics and middle-aged patients.
• Case management skills.
• Strong independent time management and organizational skills.
• Ability to set patient-centered goals.
• Skills in supervising physical therapy assistants.
• Team-oriented with professionalism and strong communication skills.
• Flexibility and a positive attitude towards providing excellent quality care.

Description :

This position involves providing home health physical therapy services.

The daily caseload is 5-6 units per therapist.

The patient population includes those with orthopedic, cardiac, neurological, pulmonary conditions, diabetes, and other diagnoses.

There will be no pediatric patients.

Therapists are required to perform various therapeutic techniques and assessments.

Successful candidates will demonstrate skills in gait training and adaptive devices.

Familiarity with the charting system is necessary.
