Job Title : Manager of Case Management

Location : Riverside, CA

Schedule : 5x8 Hour Night Shift with Rotating Weekends, 11 : 00pm to 7 : 00am

Salary Range : $142,771.20 - $214,032.00 (based on experience)

Relocation Assistance : Case-by-case basis

Job Summary :

We are seeking an experienced Manager of Case Management to lead a comprehensive care coordination program in a high-volume acute care setting. This role is responsible for overseeing case management functions, including utilization review, discharge planning, and transitions of care. The ideal candidate will demonstrate leadership in driving patient-centered outcomes, interdisciplinary collaboration, and compliance with regulatory standards.

Key Responsibilities :

• Direct and supervise case management operations, ensuring timely and appropriate patient care transitions.

Oversee utilization review, discharge planning, and care coordination processes.

Lead, mentor, and develop a team of case managers and social workers.

Collaborate with physicians, nursing, and administrative staff to improve care efficiency and patient outcomes.

Ensure regulatory compliance with CMS, Joint Commission, and other accrediting bodies.

Analyze data and trends to improve care delivery, reduce length of stay, and avoid readmissions.

Qualifications :

Active RN license in the state of California (or equivalent clinical licensure)

Bachelors Degree in Nursing, Healthcare Administration, or related field required; Masters preferred

Minimum 3-5 years of progressive leadership experience in case management within an acute care setting

Strong knowledge of utilization management, discharge planning, and regulatory compliance

Certification in Case Management (CCM or ACM) preferred
