Job Description

Job Title : Registered Nurse Case Manager

Profession : RN Case Manager

Experience : Minimum 1 year as a clinical nurse in a hospital and 9 months in home care preferred.

License : Current RN License (New York State)

Certifications : BLS, ACLS

Must-Have :

  • Certified Home Health Care Experience
  • Experience with OASIS assessments
  • EPIC or Netsmart experience preferred

Description :

This position requires the development and revision of treatment and discharge plans in conjunction with patients, families, physicians, and care delivery teams.

It involves completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in conditions within defined time frames.

The role includes processing orders and notifying physicians of patient needs and changes in condition while providing a respectful environment that upholds patient privacy and confidentiality.

Initiating appropriate preventive and rehabilitative strategies is essential.

The case manager will provide services that require specialized nursing skills and will educate patients, families, and team members regarding disease processes and self-care techniques.

Supervising LPNs to ensure patient health, safety, and compliance with care plans is required.

Directing, coordinating, evaluating, and supervising the quality of patient care services is also part of the role.

Maintaining positive relationships with patients, physicians, and referral sources is crucial for success.

The case manager will support case management philosophies and ensure compliance in clinical records with state and federal legislation.

Knowledge of available community-based services and flexibility to visit patients as needed is necessary.

Problem resolution at the point of contact must be ensured.

Participation in patient care conferences for assigned patients is expected.

The role also involves monitoring quality indicators, customer satisfaction, and pursuing efforts to eliminate avoidable costs and errors.

Responding to findings from quality management reviews and participating in peer reviews are required duties.

The case manager will maintain productivity requirements and ensure appropriate visit authorizations are in place.

Promoting a professional growth environment and participating in educational programs are essential components of this position.

Any other duties requested must also be performed as needed.
