Key Responsibilities

Provide telephonic coaching and information services to participants managing chronic health conditions

Assess participant needs and communicate with healthcare professionals for care coordination

Document participant activities and coaching sessions in case tracking software

Required Qualifications

Nursing Degree from an accredited college or university

2+ years of experience in direct participant coaching

Active and unrestricted Compact Registered Nurse License required
