Position : Geriatric Physician

Wahlen VA Medical Center, a flagship facility within the VA Healthcare System, is seeking a highly skilled and dedicated Geriatric physician to join our esteemed team of healthcare professionals. Located in the heart of Salt Lake City, Utah, our medical center is committed to providing top-tier care to our nation's veterans.

Key Responsibilities :

Patient Care :

Conduct thorough medical assessments of older adult Veterans.

Develop and implement personalized health plans tailored to the specific needs and conditions of geriatric patients.

Manage chronic diseases, cognitive decline, physical aging, and other conditions commonly affecting older adults.

Provide palliative and end-of-life care, including advance care planning and coordination with hospice services.

Interdisciplinary Collaboration :

Work closely with a multidisciplinary team of healthcare providers, including nurse practitioners, social workers, physical therapists, and dietitians, to deliver comprehensive geriatric care.

Lead and participate in case conferences and care coordination meetings.

Education and Advocacy :

Educate patients, families, and caregivers about managing health conditions, preventive care, and lifestyle modifications to improve quality of life.

Advocate for the needs of geriatric patients within the healthcare system and in the community.

Qualifications :

Board certified or board eligible.

Valid state medical license.

Strong clinical and procedural skills.

Commitment to providing exceptional patient care.

Ability to work collaboratively in a team-oriented environment.
