The Role Youll Contribute :

We are seeking a compassionate and dedicated Registered Nurse to join our Outpatient Primary Care team. In this role, you will be instrumental in delivering a high-quality care experience for our senior members. As a trusted advocate, you will build meaningful relationships with patients, their families, and caregivers, ensuring their care plans are effectively managed and coordinated across the healthcare continuum. Your commitment to excellence will support AdventHealth Senior Cares mission to provide accessible, high-touch primary care.

The Value Youll Bring to the Team :

Conduct patient triage, provide clinical advice, and monitor ongoing care.

Monitors hospitalized members and reports out at daily team huddle, completes transition of Care management calls to members within 48 hours of a hospital discharge, and schedules follow up appointments with provider.

Accurately document and execute physician and nursing orders.

Assess patient needs, including physical and emotional support, and evaluate the availability of their support systems.

Administer treatments and medications, wound care, various point of care testing, respond to clinical emergencies, and evaluate the effectiveness of interventions.

Provide patient education tailored to individual care plans and age-specific needs.

Participate in performance improvement initiatives and support departmental quality standards through accurate documentation and continuous monitoring.

The expertise and experiences you'll need to succeed :

Minimum qualifications :

Bachelor's of Nursing

1 year nursing experience

Registered Nurse (RN)

Driver's License (DL)

Basic Life Support - CPR Cert (BLS)

Preferred qualifications :

• Master's of Nursing
