We are seeking a dedicated and compassionate Family Nurse Practitioner to join our healthcare team. The ideal candidate will provide patient assessments and diagnoses. The Adult Nurse Practitioner will work collaboratively within a team and collaborate with physicians, nurses, therapists, and families.

Duties

• Conduct comprehensive assessments of patients to determine their healthcare needs.
• Develop and implement individualized care plans tailored to each patient's unique circumstances.
• Offer ongoing support and education to patients and their families regarding health management and disease prevention.
• Collaborate with interdisciplinary teams to ensure excellent patient care.
• Document patient interactions accurately in the electronic health record system.
• Engage in continuous learning and professional development to stay current with best practices in medicine.
