Schedule : Monday-Friday | 8 : 00 AM-4 : 30 PM

Position Overview

Dean's Professional Services is actively seeking an experienced Case Manager RN to support hospital-based operations in Houston, TX. This role is responsible for assisting the Director of Case Management with the implementation and daily management of the inpatient case management program. The Case Manager RN plays a critical role in care facilitation, utilization management, discharge planning, and quality improvement initiatives while ensuring the appropriate and effective use of hospital resources.

Key Responsibilities

Assist the Director of Case Management in implementing and managing the inpatient case management program.

Oversee core program components including Care Facilitation, Utilization Management, Case Management, and Discharge Planning.

Coordinate hospital-level systems and processes related to care and utilization management.

Support department activities related to discharge planning and clinical quality improvement.

Manage day-to-day departmental operations and ensure appropriate use of hospital resources.

Participate in the collection, analysis, and reporting of financial, utilization, quality, and performance improvement data.

Collaborate with interdisciplinary teams to support safe, efficient patient transitions of care.

Qualifications

Active, unrestricted Registered Nurse (RN) license in the state of Texas

Previous case management experience in an inpatient hospital setting

Strong understanding of utilization management, discharge planning, and care coordination

Excellent communication, organization, and leadership skills

Ability to analyze quality and financial performance data
