We are seeking a Permanent Primary Care Physician in California

Position Type : Full-Time

Specialty : Physician Primary Care (Internal Medicine, Family Medicine, or Geriatrics)

Location : Fresno, California

Rate : Open & Negotiable

Shift : Monday Friday, standard business hours; rotating night / weekend call

Start Date : ASAP

End Date : Ongoing

Position Details :

Patient Population : Geriatric (PACE participants, typically 55+)

Setting : Community-based outpatient, nursing homes, and hospitals

Responsibilities :

Conduct in-person comprehensive assessments

Participate in IDT meetings to develop and review individualized care plans

Provide and oversee medical care in clinic, hospital, and nursing home settings

Collaborate with other providers and support teams to ensure continuity of care

Provide direction to Nurse Practitioners and team members

Participate in rotating on-call coverage

Required Procedures : General outpatient / geriatric primary care procedures

EMR System : Not specified

Support Staff : Interdisciplinary Team (IDT) including Nurse Practitioners and care managers

Days Per Month : Full-time schedule

On-Call / Weekends : Rotating night and weekend call

Submission Requirements :

Active California Physician License

Board Certification in Internal Medicine, Family Medicine, or Geriatrics (preferred)

Completion of ACGME-accredited residency

Active DEA and state CDS certification

Eligible for Medicare and Medi-Cal enrollment
