Conduct thorough speech and language evaluations and develop personalized care plans and treatment goals.Assess and treat patients with speech, language, voice, and swallowing disorders as prescribed by a physician.Regularly communicate with physicians to update treatment plans and address any ongoing patient issues.Maintain accurate clinical records according to state and federal regulations.Manage a caseload effectively, coordinating services and advocating for optimal patient outcomes.Conduct ongoing assessments and modify care plans in response to patient progress.Educate patients and their caregivers on therapy procedures and supportive activities.Oversee patient discharge planning, including providing home programs and referrals for community services.Demonstrate age-specific competency in care provision across the lifespan.Licensure, Registration, and / or Certification Required :
Speech-Language Pathology license issued by the state of WisconsinBasic Life Support (BLS) certification from the American Heart Association (to be obtained within 6 months)A valid driver's licenseEducation Required :
Master's Degree in Speech PathologyExperience Required :
Typically 3 years of experience in speech therapy in an acute hospital or home health care settingKnowledge, Skills & Abilities Required :
Proficiency in clinical skills, with the ability to work independentlyIntermediate computer skillsExceptional communication and interpersonal skillsStrong organizational, decision-making, and problem-solving abilitiesPhysical Requirements and Working Conditions :
