Job Description

We are seeking a compassionate and skilled Speech Therapist to join our dynamic home health team. The ideal candidate will provide high-quality speech, language, and swallowing therapy services to patients in their homes, helping them achieve their communication and functional goals.

Responsibilities :

• Conduct comprehensive assessments of patients' speech, language, cognitive-communication, and swallowing abilities.
• Develop individualized treatment plans tailored to patients' needs and goals.
• Provide therapy to improve speech articulation, language skills, voice, fluency, and swallowing function.
• Educate patients and families on strategies to enhance communication and manage swallowing difficulties.
• Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to ensure holistic patient care.
• Document patient progress and maintain accurate records in compliance with regulatory standards.
• Adhere to home health safety protocols and infection control measures.
