Mend Health is seeking a skilled and compassionate Mobile Wound Care Nurse Practitioner to join our growing team. As a Nurse Practitioner specializing in wound care, you will play a crucial role in delivering high-quality healthcare services to our patients in various home settings. You will be responsible for assessing, treating, and managing acute and chronic wounds, collaborating closely with patients, families, and our multidisciplinary team to promote healing and optimize patient well-being.

Key Responsibilities :

Conduct comprehensive wound assessments, including wound measurements, tissue viability assessment, and identification of underlying etiologies.

Develop and implement individualized wound care plans based on assessment findings, evidence-based practices, and patient preferences.

Perform wound debridement, dressing changes, and wound care procedures using sterile techniques and ensuring patient comfort and safety.

Educate patients, caregivers, and families on wound care management techniques, preventive measures, and signs of wound complications.

Collaborate with physicians, specialists, and other healthcare providers to coordinate patient care and optimize treatment outcomes.

Maintain accurate and up-to-date medical records, including wound assessment documentation, treatment plans, and progress notes.

Participate in continuous education and training to stay current with advancements in wound care management and technologies.

Adhere to Mend Health's policies, procedures, and regulatory requirements, ensuring compliance with standards of practice and patient safety protocols.

Contribute to the leadership team and play a significant role in the Company's growth strategy

Qualifications :

Master's degree in Nursing from an accredited Nurse Practitioner program.

Active, unrestricted Certified Nurse Practitioner (CNP) license in the state of practice, with national certification as a Nurse Practitioner in Adult-Gerontology, Family, or Acute Care.

Certification in Wound, Ostomy, and Continence Nursing (CWOCN) preferred.

Minimum of 3-5 years of clinical experience as a Certified Nurse Practitioner (CNP).

Specialization in wound care preferred.

Proficiency in performing wound assessments, wound care procedures, and wound dressing techniques.

Strong interpersonal and communication skills, with the ability to effectively educate and empower patients and caregivers.

Ability to work independently and collaboratively within a multidisciplinary team environment.
