Pinnacle Home Care, Florida's largest Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we're looking for Full Time, IV Certified, Licensed Practical Nurses (LPN) to join our award-winning team.

Key Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.
• Utilize clinical expertise and nursing education to make informed, independent decisions that support patient care and promote positive outcomes.
• Complete visit reports promptly and accurately within the electronic medical record system to support high-quality care and regulatory compliance, including all required OASIS documentation.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Demonstrate awareness of the agency's Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Understand and adhere to the agency's process for managing and addressing customer complaints.
• Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency's service agreements and admission policies.
• Understand the agency's policies regarding appropriate patient discharge and reasons for discharge.
