Optum is seeking a Nurse Practitioner or Physician Assistant to join our HouseCalls team in Brooklyn, NY. Optum is a clinician-led care organization, that is creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you'll provide annual clinical assessments to patients in the comfort of their homes. This important preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow-up care to improve health and well-being.

This HouseCalls role will require 80% travel and overnight trips with hotel stay. The NP / PA must travel by vehicle. Counties include : Brooklyn, Queens and Westchester.

In "HouseCalls" we provide annual clinical assessments (similar to physical exam) to patients in the comfort of their own homes. This important preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow-up care to improve health and well-being. You will be traveling on site each work day to patient residents and all members have Medicare Advantage plans. We do the scheduling for you!

The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
