Wisconsin Licensed RN Care Manager

A company is looking for a Registered Nurse Care Manager.

Key Responsibilities

Conduct comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs

Create and implement care plans in collaboration with patients and the care team

Coordinate care across various settings and manage transitions to minimize avoidable readmissions

Required Qualifications

1+ years of nursing experience in case management or care management

2+ years of healthcare-related experience

Current, unrestricted compact Registered Nurse license

Current BLS certification from a licensed AHA or American Red Cross training facility
