Responsibilities :

• Work Tuesday - Saturday weekly
• Conduct initial and ongoing assessments of patients' health status and develop individualized care plans
• Administer medication, treatments, and other nursing interventions as prescribed by the patient's physician
• Monitor patients' conditions and provide necessary education and support to help them manage their health
• Document all patient care activities in accordance with company policies and regulations
• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care for patients
• Communicate regularly with patients, families, and caregivers to provide updates on the patient's progress and address any concerns
