Overview :
Sentara is hiring a Clinical Pharmacy Specialist for our Population Health Team
Location : Sentara Park- Virginia Beach, VA
Status : Full Time (40 hrs / wk)
Shift : Day
Hours : 8am-5pm
- This Hybrid position requires 2 days per week in office at Sentara Park, days on site at affiliate practices throughout Tidewater Virginia and allows for some work from home days
The Clinical Pharmacy Specialist participates in all aspects of the medication-use system while delivering comprehensive and personalized pharmaceutical care to patients in their designated areas. This role assumes leadership and accountability for specific clinical pharmacy areas, demonstrates advanced skills and expertise, and obtains advanced certification in their practice area if available.
Manages patient drug therapy in accordance with applicable clinical practice standardsAssures the appropriateness of all provider medication orders in maintaining a high level of patient care and safetyMaintains advanced knowledge of medication use, disease states, and regulations applicable to their area of practiceServes as the primary resource for drug information and educationProvides in-depth patient counseling when requiredSupports the training of new employees and participates in the creation and maintenance of training materialsAssumes local and system responsibility and ownership for assigned tasksConsistently follows and assists with the development of policies and proceduresDemonstrates superior customer service, effective communication, decision-making, and organization to ensure efficient job performance and successEnsures adequate supplies are available for continuous patient careTriages and escalates any questions or issues outside the scope of practice as neededUses appropriate systems for processing of patient informationEnsures quality assurance is maintained according to departmental and legal requirementsPrecepts pharmacy students and / or pharmacy residents as assignedProvide peer coaching and constructive feedback to other colleagues in a professional and productive mannerSuccessfully leads local and system implementation of various system initiativesServes as a system expert and site lead in specialized clinical pharmacy functionsIn addition to the above job responsibilities, other duties may be assignedEducation
Doctorate Degree in Pharmacy (Required)Certification / Licensure
Virginia Pharmacist License (Required)Basic Life Support (BLS) (Required)Advanced Cardiac Life Support (ACLS) (Required)BCPS or BCACP- must have within 3 years of hire (Required)Experience
