We are seeking an experienced and compassionate Part-Time / Per Diem Physical Therapist (PT) to join our home health care team. As a PT, you will provide individualized physical therapy services to patients in their homes, helping them regain mobility, strength, and independence after illness, injury, or surgery. You will collaborate with a multidisciplinary team to ensure that each patient receives personalized care and achieves their rehabilitation goals.

Key Responsibilities :

Perform comprehensive patient evaluations to assess mobility, strength, balance, and functional limitations.

Develop and implement individualized treatment plans to improve physical function, mobility, and overall quality of life.

Provide hands-on therapy, including therapeutic exercises, manual techniques, and other interventions to restore movement and function.

Educate patients and their families on safe mobility techniques, home exercises, and strategies to prevent injury or complications.

Monitor and document patient progress, adjusting treatment plans as necessary to meet therapy goals.

Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.

Maintain accurate and timely documentation of patient evaluations, treatments, and progress using the electronic medical record (EMR) system.

Ensure compliance with agency policies, procedures, and state / federal regulations for home health care.

Qualifications :

Active and unrestricted Physical Therapist (PT) license in [State].

Minimum of 1 year of clinical experience, preferably in home health, rehabilitation, or a related field.

Strong knowledge of physical therapy principles and home health care regulations.

Proficiency in electronic medical record (EMR) systems.

Excellent communication and interpersonal skills.

Ability to work independently and manage a diverse patient caseload effectively.

Compassionate and patient-focused, with a commitment to improving patient outcomes.

Valid driver’s license and reliable transportation.
