Tennessee Licensed Recovery Partner

A company is looking for a Recovery Partner (REMOTE).

Key Responsibilities

Leads communication and coordination of care among the care team members

Supports recovery care coordinator and social services during the monitoring phase

Coaches patients in self-management and arranges for closure of gaps in care

Required Qualifications, Training, and Education

High school diploma or equivalent

Willing to learn and practice motivational interviewing during patient interactions

Willing to learn a self-management model for patient support

Preferred : EMT-B certification

Preferred : Medical professional background such as medical assistant or health coach
