HealthPro Heritage is seeking a dedicated Occupational Therapist to join our rehab team at Sage Post Acute in Hayward, CA. This role focuses on providing high-quality therapy services to residents across Skilled Nursing (SNF), Outpatient, and Transitional Care , helping patients regain independence and achieve their goals.
