Occupational Therapist

Job Summary :

The Occupational Therapist provides skilled occupational therapy services to patients in their homes, focusing on improving their ability to perform daily living activities. This role involves patient assessments, treatment planning, and collaboration with an interdisciplinary team to optimize patient outcomes. The therapist will travel to patients' homes within a designated service area, providing individualized care and support.

Location : Medford, Oregon, United States

Responsibilities :

• Conduct patient assessments to determine occupational therapy needs and develop appropriate treatment plans.
• Provide skilled occupational therapy interventions to improve patients' functional abilities and independence.
• Educate patients and their families on therapeutic techniques and home exercise programs.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Maintain accurate and timely documentation of patient assessments, treatment plans, and progress notes using EPIC / Oasis charting system.
• Manage a caseload of patients, optimizing travel routes to maximize efficiency and productivity.
• Adhere to all facility policies and procedures, including infection control and safety guidelines.
