Duties

• Collaborate with healthcare team members to develop and implement individualized care plans.
• Document patient information accurately in electronic health records (EHR) systems.
• Educate patients and their families on health management and disease prevention strategies.
• Participate in triage and emergency response situations as needed.
• Maintain compliance with HIPAA regulations and infection control protocols.
• Support patients with special needs, including those with developmental disabilities or Alzheimer's disease.
