RN Care Manager Field-based - Brunswick

This is a field-based position that requires travel to patients' homes in or around Brunswick, GA.

Member engagement and management. Engagement can include : care management, member outreach via phone and community events.

Work Monday - Friday, day shifts only!

No Major Holidays!

No On Call!

How You'll Make a Difference :

Arkos Health is seeking a RN Care Manager. The RN Care Manager will be responsible for working with patients and their families to ensure a smooth transition following discharge from any facility. In addition, the Nurse CM provides transitional and intensive nurse care management services for the entire network and works collaboratively with the health plan, leadership, department coordinators to ensure the highest quality of patient satisfaction is met.

This care management role requires regular visits to patients in their homes as well as telephonic outreach. While experience in these areas may be beneficial, this role is not worker's comp, hospice, or home health
