About the Role

For one of our client in healthcare, which is a mission-driven organization delivering comprehensive, compassionate care to the elderly and medically vulnerable, we are seeking a dedicated and experienced Social Work Manager to lead their social services and behavioral health team.

In this vital leadership role, you will oversee a team of Social Workers, Behavioral Health Specialists, and a Social Work Supervisor—ensuring high-quality care that aligns with each participant’s individualized care plan.

What you'll be doing

• Lead and supervise a multidisciplinary team including Social Workers, Behavioral Health Specialists, and a Social Work Supervisor.
• Oversee the quality and consistency of psychosocial assessments, care coordination, and participation in Interdisciplinary Team (IDT) meetings.
• Ensure timely and effective delivery of social work services in response to participant needs.
• Support professional development and performance of the social work team.
• Collaborate closely with clinical leadership to improve care models and participant outcomes.
