Salary : $95,000 - $105,000 annually

Join our mission-driven Managed Long Term Care (MLTC) program dedicated to serving the diverse communities of Brooklyn as an Intake Registered Nurse . This fulfilling hybrid role involves conducting field-based assessments and collaborating with our in-office team.

As an Intake RN , you will be essential in assessing potential new members for program eligibility, providing them and their families with valuable information about available services, and ensuring a seamless transition into long-term care. Bilingual skills in Chinese or Spanish are highly advantageous to effectively serve our community.

Key Responsibilities :

Perform in-person assessments using the UAS-NY tool to evaluate program eligibility and ensure safety for prospective members.

Educate potential members and their families about the MLTC program, its services, and the enrollment process.

Collaborate with our Intake / Outreach and Clinical teams to facilitate the timely and appropriate onboarding of new members.

Maintain professional communication with referral sources, physicians, caregivers, and community partners.

Engage in outreach activities to enhance community connection and referrals.

Qualifications :

Active New York State RN license is required.

Bachelor's degree in Nursing is necessary.

A minimum of 1 year of acute care experience is required.

Experience with UAS-NY assessments is mandatory.

Previous experience in home care and care management is preferred.

Strong communication and interpersonal skills; fluency in Spanish or Chinese is highly desirable.

Proficiency in Microsoft Office applications is expected.
