About the Role
In this role, you will conduct comprehensive assessments during visits that last up to one hour. Our assessments cover a member's health, medical history, medication adherence, and social environment, providing insights essential for improving care quality. Your responsibilities will include :
- Conducting adult and geriatric assessments, including documentation according to Matrix guidelines
- Administering and ordering lab-based and point of care testing as necessary
- Collaborating with a multidisciplinary team, including physicians and social workers
- Educating members and coordinating follow-up care with their Primary Care Physician
- Providing services in members' homes or Skilled Nursing Facilities

Master's Degree or equivalent experience with successful NP or PA licensureCurrent NP or PA licensure with prescriptive authority in the state of practiceBoard certification in a relevant specialty (e.g., Adult, Family, Emergency)Current BLS, ACLS, or CPR certificationPreferred but not required : 1 year of experience as a Nurse Practitioner or Physician Assistant; new graduates are encouraged to apply!Valid driver's license and proof of auto insuranceStrong computer skills and familiarity with health / medical record softwareExcellent verbal and written communication skillsThe Matrix Culture
