About the Role

In this role, you will conduct comprehensive assessments during visits that last up to one hour. Our assessments cover a member's health, medical history, medication adherence, and social environment, providing insights essential for improving care quality. Your responsibilities will include :

• Conducting adult and geriatric assessments, including documentation according to Matrix guidelines
• Administering and ordering lab-based and point of care testing as necessary
• Collaborating with a multidisciplinary team, including physicians and social workers
• Educating members and coordinating follow-up care with their Primary Care Physician
• Providing services in members' homes or Skilled Nursing Facilities
