Job Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. As an LPN, you will work under the supervision of registered nurses and physicians, assisting in the delivery of patient care while ensuring compliance with healthcare regulations and standards.

Duties

  • Provide direct patient care, including administering medications and treatments as prescribed.
  • Conduct patient assessments and monitor vital signs to evaluate patients' health status.
  • Assist in the development and implementation of individualized care plans.
  • Perform procedures such as IV insertion, catheterization, and wound care.
  • Maintain accurate medical documentation and records in compliance with HIPAA regulations.
  • Collaborate with healthcare team members to ensure effective communication and continuity of care.
  • Educate patients and their families on health management and disease prevention.
  • Support patients with activities of daily living (ADLs) and mobility assistance.
  • Participate in discharge planning and follow-up care coordination.
