Purpose : In alignment with a multi-disciplinary health team, the Community Health Specialist I is responsible for providing sexual health services, testing, and health education to diverse populations, especially those at increased risk of health disparities.
Key Responsibilities :
- Programming : Schedule, plan, and present culturally competent educational and risk reduction programming and HIV / STI / HCV prevention and treatment education to individuals and groups. Ensure PrEP / PEP and HIV, HCV, and STI prevention are integrated into programming and outreach programs. Assist with the development and implementation of new and established programs / education and outreach sites.
- Service Delivery : Participate in outreach and link contacts to appropriate care and / or services. Operate assigned vehicles, including outreach mobile unit. Identify patients appropriate for PrEP / PEP and provide linkage to care services. Facilitate linkage to additional services as needed.
- Testing : Provide HIV / HCV and STI screening and testing services. Discuss strategies for risk reduction and assist clients in creating practical risk reduction strategies. Connect clients to services and care, including post-and Pre-exposure Prophylaxis.
- Administrative : Compile and report program and service data. Participate in training and continuing education initiatives. Navigate Microsoft Office products and other electronic platforms for communication and documentation.
- Community Networking : Represent the agency at community events. Participate and provide representation on relevant coalitions, committees, networks, etc.
- Miscellaneous : Respect and support inclusiveness and diversity. Maintain confidentiality of all patients, clients, employee, protected, and proprietary information. Participate in compliance audits, process improvement, and quality improvement plans.
Qualifications :
High School Diploma or equivalent and four years of experience in Health Education, Health and Human Services, or a related field.Associate’s degree in health education, Health and Human Services, or related field and 2 years of related experience.Bachelor’s degree in health education, Health and Human Services, or related field (preferred).Excellent verbal and written English communications skills required.Bilingual in Spanish and / or ASL desired.Physical Requirements :
Ability to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally stoop, bend and lift or move up to 25 lbs.
